MSC Form 783 (Feb 65) “Special Run Request,” filled out by hand by TRW’s Hinely on July 24 1971, two days before Apollo 15’s July 26 launch: Project No. 1570C, a 45-line special computer run needed “ASAP” for “mission” support, stamped “APOLLO 15” and “PRIORITY” in red and purple ink to flag it for expedited processing (faint corner stamps appear to read received/completed dates of July 26 and 27).
TRW’s Mission Planning and Analysis Division held Manned Spacecraft Center support contracts for Apollo trajectory reconstruction and guidance and control analysis, the kind of rush job this slip was requesting.
The request form is printed on the front of a standard 80-column Column Binary IBM card; the reverse retains the card’s unused, unpunched pre-printed layout, whose header fields (TAG, SUM, CHECK, INITIAL LOCATION, WORD COUNT, RELOC) match a CDC binary card format used to load object code into the Control Data Corporation mainframes contractors like TRW used for trajectory work.
